CRP and ferritin answer different questions. C-reactive protein (CRP) measures active inflammation. Ferritin measures the body’s iron stores. The complication is that ferritin is also an acute-phase reactant, so infection or inflammation can raise it even when iron stores are normal. What is a CRP test?
CRP is a protein the liver releases as part of the acute-phase response. Levels rise within hours of infection, tissue injury, or an inflammatory flare, and fall quickly once the trigger resolves. That fast rise and fall makes CRP useful for evaluating and monitoring infections, inflammatory conditions, tissue injury, and response to treatment.
CRP is nonspecific: an elevated result signals that inflammation may be present, but not where it is or what caused it. Labs commonly report CRP in mg/L or mg/dL, where 1 mg/dL equals 10 mg/L. What is a ferritin test?
Ferritin is the body’s iron-storage protein, and a blood ferritin level is the standard routine test of iron stores. Low ferritin is the most specific routine blood finding of iron deficiency. Ferritin is reported in ng/mL, which is equivalent to µg/L.
A high ferritin is harder to read. Because ferritin rises with inflammation regardless of iron status, an elevated result needs iron studies, such as transferrin saturation, and inflammation context before it can be attributed to iron overload.

The pairing earns its place in two situations.
First, a high ferritin: checking CRP alongside it helps decide whether the elevation reflects inflammation rather than iron overload. Second, a normal ferritin in someone who is inflamed: an elevated CRP flags that the ferritin may be masking iron deficiency, since inflammation pushes ferritin up from a genuinely low baseline.

- Do you need to fast? No. You don’t need to fast for CRP or ferritin on its own. Ferritin is sometimes drawn with iron studies that may have their own instructions, so follow the instructions on your order.
- Can both be tested from one blood draw? Yes. They are commonly ordered together, and the CRP result helps interpret the ferritin.
- Does a high ferritin mean iron overload? Not by itself. Infection or inflammation can raise ferritin independent of iron stores, so an elevated result needs the workup described above before it is read as overload.
