What High Vitamin B12 Means

B12 is stored primarily in the liver and released into circulation as needed. Very high serum B12 — above the reference range — from supplementation reflects excess intake that exceeds storage and renal excretion capacity. In the absence of supplementation, high B12 can indicate that B12 is being released from damaged liver cells, or that a myeloproliferative disorder is producing excess transport proteins (haptocorrins) that bind and elevate measured B12 levels.

Symptoms of High Vitamin B12

From supplementation: usually no symptoms. Very high injection doses occasionally cause acne-like skin eruptions.

Unsupplemented high B12 produces symptoms of the underlying condition rather than B12-specific effects:

  • Liver disease: jaundice, abdominal discomfort, fatigue (from hepatitis or cirrhosis)
  • Myeloproliferative disorders (polycythemia vera, chronic myeloid leukemia): fatigue, spleen enlargement, headaches, itching after showering, visual disturbances

Is high B12 from supplements dangerous?

Not in most cases. B12 is water-soluble and excess amounts are excreted in urine. There is no established upper tolerable intake level for B12 because toxicity from supplementation is extremely rare. Very high injection doses have been associated with acne in some individuals.

The liver stores roughly 50 to 90% of the body’s total B12. When liver cells are damaged (by hepatitis, alcohol, or cirrhosis), they release their stored B12 into the bloodstream, raising serum levels. High B12 can therefore be an early signal of hepatocellular damage.

What is a myeloproliferative disorder?

These are bone marrow conditions where one cell line proliferates excessively — polycythemia vera (red cells), essential thrombocythemia (platelets), or chronic myeloid leukemia (white cells). They produce excess haptocorrins (B12 transport proteins), which elevate measured serum B12.

Should I stop B12 supplements if my level is high?

If you are supplementing and have no symptoms or concerning findings on other labs, a high B12 is usually not a concern. You may reduce the dose if you are taking very high amounts. Retesting after stopping supplements for 4 to 6 weeks can clarify whether supplementation is the sole driver.
